HIPPA

Google Forms can be HIPPA compliant by signing the BAA. I've included a link in case you need it.

The following Google products are included in the HIPPA Functionality - The following functionality is Included Functionality under the applicable HIPAA Business Associate Addendum as of July 2021:

Gmail, Calendar, Drive (including Docs, Sheets, Slides, and Forms), Apps Script, Keep, Sites, Jamboard, Hangouts (chat messaging feature only), Google Chat, Google Meet, Google Voice (managed users only), Google Cloud Search, Cloud Identity Management, Google Groups, Google Tasks and Vault (if applicable).

Third party integration are not covered under Googles HIPPA functionality and must have their own. None of my templates include third-party data requesting, so you're covered by this. 

This applies to Workspace account users not Personal Google Site Users.
